What it costs, and what moves the number

Indicative ranges, the clinical variables behind them, and a plain list of what a quotation does and does not cover. Nothing here is a price list — under Turkish health advertising rules, and in good practice, a price follows an assessment rather than preceding it.

Men researching treatment abroad are usually comparing three or four clinics whose published figures differ by a factor of two, with no way to tell what accounts for the gap. Often the gap is the device. Sometimes it is the hospital. Occasionally it is the surgeon’s experience, which is the one variable that should never be the cheap component.

The ranges below are indicative and given in euros for comparability; quotations are issued in euros, US dollars, pounds sterling or Turkish lira. They cover the whole package — surgery, hospital, device, hotel, transfers, interpreting and follow-up — not the surgical fee alone.

Where the variation comes from

Six things that move a quotation

The device

Three-piece inflatable prostheses from Coloplast and Boston Scientific sit at the top of the range; two-piece and malleable devices cost materially less. The choice is clinical first — anatomy, dexterity, prior surgery — and financial second.

Complexity of the case

Virgin corpora with normal tissue take less theatre time than fibrosis after priapism, prior implant removal or radical prostatectomy. Corporal dilation in scarred tissue can double operative time and occasionally requires additional instrumentation.

Combined procedures

Straightening manoeuvres for Peyronie’s, circumcision or a girth procedure carried out in the same anaesthetic add cost but far less than a second trip would — and a single recovery period rather than two.

Length of stay

Most implant patients need four to seven nights in Istanbul. Revision and salvage pathways run longer. The hotel and transfer element of the package scales with that, which is why nights are stated on every package page.

Comorbidity management

Poorly controlled diabetes, anticoagulation or cardiac disease may require additional pre-operative assessment or an anaesthetic review. Occasionally it means postponing until control improves — a clinical decision, not a commercial one.

Filler volume

Girth enhancement is priced by the volume of hyaluronic acid used, and by whether it is delivered in one session or two. The volume needed is estimated at consultation and confirmed on examination.

A note on comparison

How to compare two quotations honestly

Ask each clinic the same six questions and write the answers side by side. Who performs the surgery, and how many of these procedures do they do in a year? Which device, from which manufacturer, and will the serial number be registered in my name? Is the hospital accredited, and by whom? What exactly does the price include, and what does it exclude? What are the written revision terms, and for how long do they run? Who is responsible for me in the twelve months after I fly home?

A clinic that answers all six in writing is easy to compare. One that answers none of them is also telling you something, and the price is the least of it.

Questions about cost and payment

Why do you not publish a single fixed price?

Because the two largest components of the cost — the device selected and the complexity of the case — are only known after a urologist has read your history. A first implant in a healthy man and a revision on scarred corpora are different operations with different theatre times, device requirements and risk profiles. Quoting one number for both would mean overcharging one patient and misleading the other.

Is the written quotation fixed?

Yes, provided your clinical picture matches what you described and what your records show. If assessment in Istanbul reveals something materially different — unexpected corporal fibrosis, an infection requiring a staged approach, a cardiac finding that changes the anaesthetic plan — we stop, explain it and re-quote before proceeding. We do not vary a price during a procedure, and we do not add charges afterwards.

Why is treatment in Istanbul less expensive than in western Europe or North America?

Chiefly staffing, facility and insurance costs, plus the exchange rate — not lower standards of care. The implant device itself is the same manufacturer part number at a comparable price worldwide; what differs is everything around it. High case volume in a small number of centres also reduces theatre time per case, which is both a cost and a quality argument.

How and when do I pay?

A deposit confirms your surgical date and, for implant surgery, reserves the device. The balance is settled at the clinic before the procedure, by card or bank transfer. You will receive an invoice in your own name. Cash payment is never required, and nobody at the clinic should ever ask you for one.

What happens financially if a complication occurs?

Your treatment agreement sets out the revision terms in writing before you travel, including which scenarios are covered without a further surgical fee and over what period. Read that section carefully and ask about anything you find ambiguous — a clinic that will not put revision terms in writing before you fly is telling you something.

Do you work with insurers?

Penile prosthesis surgery is reimbursed by some insurers and national systems where erectile dysfunction is refractory to medical therapy, and refused by others. We supply itemised invoices, the operative report and a medical necessity letter so you can submit a claim, but we cannot bill an overseas insurer directly and reimbursement is between you and them.

Should I choose a clinic on price?

Not on price alone. Ask who performs the surgery and how many of these procedures they do a year, which device brands are used and whether the serial number is registered in your name, whether the hospital is accredited, what the revision terms are, and who is responsible for you in the first twelve months. A quotation is only comparable once you know what sits behind it.
