Girth enhancement injects cross-linked hyaluronic acid into the loose tissue plane beneath the skin of the penile shaft, increasing circumference immediately. It is performed under local anaesthetic block, takes under an hour, and is fully reversible because hyaluronic acid can be dissolved with hyaluronidase.

How filler augmentation works
Beneath the skin of the penile shaft lies a loose, mobile layer — the dartos fascia — that slides freely over the deeper structures. That plane is what allows the skin to move during erection, and it is the only correct place for filler. Cross-linked hyaluronic acid is deposited evenly into it through a blunt-tipped cannula, adding a uniform layer of volume around the whole circumference of the shaft.
Because the filler sits above the tunica albuginea and outside the erectile bodies, it does not interfere with the mechanism of erection, does not touch the urethra, and does not alter sensation from the dorsal nerve. The penis becomes thicker in both flaccid and erect states, and remains fully functional.
Hyaluronic acid is a sugar molecule that occurs naturally in human connective tissue. The cross-linked forms used here are gradually broken down by the body over twelve to twenty-four months. That impermanence is a feature rather than a flaw: nothing is left behind that cannot be removed, and if you dislike the result it can be dissolved within days using hyaluronidase.
Girth enhancement options compared
Several materials have been used to thicken the penis. We offer hyaluronic acid because its risk profile and reversibility are, in our judgement, the only ones acceptable for an elective cosmetic procedure on this anatomy.

What the session is like
- Consultation and measurement Flaccid and erect circumference are measured at three points along the shaft and documented. We agree a target volume and, where the target is large, plan it across two sessions rather than overloading the tissue in one.
- Anaesthesia Topical anaesthetic cream, then a dorsal penile nerve block. The block is the only genuinely uncomfortable part of the procedure and takes seconds.
- Injection Filler is delivered through one or two small cannula entry points using a fanning technique, with the shaft massaged continuously to keep distribution even. Twenty to forty minutes depending on volume.
- Immediately after You see the result on the table. There is mild swelling on top of the true result, so the shaft looks slightly fuller on day one than it will at week four. You walk out and can eat, travel and work normally the same day.
- Days 1–14 Light bruising at the entry points, occasional mild swelling of the foreskin. We ask you to massage the shaft daily to a prescribed pattern, avoid cycling and gym work for two weeks, and abstain from sexual activity.
- Week 3–4 Swelling has fully settled and the result is final. Intercourse resumes. A review photograph and measurement complete the record.
What you can realistically expect
- A gain of roughly 1 cm in circumference for every 6–8 ml of filler, in a typical adult shaft.
- A 20 ml session commonly produces 2.5–3 cm of added circumference — a visibly and palpably thicker shaft.
- The gain applies to both the flaccid and erect states, and is proportionally more striking flaccid.
- Filler is placed on the shaft only. Glans augmentation is a separate discussion and is not performed at the same sitting.
- Around 60–70% of the volume typically remains at twelve months. A smaller top-up session then restores the original result.
- Erection quality, sensation, urination and ejaculation are unaffected.
Risks and how they are managed
- Swelling and bruising. Universal, mild, and resolved within two weeks.
- Nodules or irregularity. The commonest genuine complication, usually from uneven distribution or from massaging too little. Most respond to massage; the remainder are dissolved locally with hyaluronidase.
- Asymmetry. Corrected with a small top-up at review rather than by reinjecting on the day.
- Migration. Uncommon with correctly placed, appropriately cross-linked filler in the dartos plane; more likely if too much volume is placed in one session.
- Infection. Rare, and minimised by full sterile technique and a single entry point. Treated with antibiotics and, if necessary, dissolution of the filler.
- Vascular occlusion. Very rare but the reason this must never be done by a non-medical injector. Blunt cannulas, aspiration and knowledge of the dorsal vascular anatomy are the safeguards, and hyaluronidase is kept in the room.
Questions we are asked about girth enhancement
Is the result permanent?
No, and deliberately so. Hyaluronic acid is broken down by the body over roughly twelve to twenty-four months. Most men return for a smaller maintenance session after the first year, at which point the cumulative result tends to hold for longer.
Can the filler be removed if I do not like it?
Yes. Hyaluronidase, an enzyme injected in the same way as the filler, dissolves hyaluronic acid within a day or two. This is the single strongest argument for choosing it over permanent materials, and it is why we do not offer permanent gels.
Will it affect my erections or sensation?
It should not. The filler is placed in the loose layer under the skin, well outside the erectile bodies and away from the urethra and the dorsal nerve. Erection mechanics, sensation, orgasm and urination are unchanged.
How much filler do I need?
Most patients choose between 10 ml and 30 ml. The right volume depends on your starting circumference, skin laxity and what you want to achieve, and it is decided at consultation with measurements taken. Where the target volume is high we stage it over two sessions three months apart, because overfilling a single session is the main cause of nodules.
When can I have sex again?
Three to four weeks. The filler needs that time to integrate and distribute evenly; earlier activity is the commonest cause of displacement and irregularity.
Can this be combined with lengthening surgery?
Yes, and it frequently is. When both are planned, lengthening surgery is performed first and filler is added once the surgical swelling has fully settled, usually at eight to twelve weeks. Doing them the other way round makes the surgical dissection harder.
Can I have filler if I already have a penile implant?
Usually yes, provided the implant is well healed and free of infection. The planes are different and the two do not interfere. We would want at least six months from implant surgery, and we will confirm after reviewing your operative notes.
